Two Case Studies Presented at IHI National Forum Demonstrate Use of Glytec’s Technology and Services Leads to Improved Glycemic Outcomes

AdventHealth Waterman and Sentara Virginia Beach General Hospital showcased reductions in hypoglycemia and glycemic practice variation utilizing Glytec’s comprehensive systems approach to glucose management.

In partnership with clinical leaders from AdventHealth Waterman and Sentara Virginia Beach General Hospital, Glytec presented two storyboards at the IHI National Forum revealing significant quality and safety improvements for hospitalized patients receiving insulin therapy. On average, this population represents as many as one of every three patients admitted to a general ward and one of every two patients admitted to an intensive care unit. Both hospitals credit their collaboration with Glytec, including utilization of the company’s FDA-cleared decision support platform, eGlycemic Management System® (eGMS®), as well as the company’s highly-tailored quality improvement services.

With Glytec, Hospital Moves to Basal-Bolus Insulin, Saves $9.7 Million

The move to quality ratings will put more pressure on hospitals to find ways to improve the standard of care while cutting costs.

A decade ago, the American Diabetes Association (ADA) called for hospitals to shift from sliding scale to subcutaneous basal-bolus insulin therapy for non-critically ill patients, but change has been slow in coming. Fear of hypoglycemia and concerns that staff would not be able to keep up with dosing demands of basal-bolus kept hospitals from upgrading to the standard of care.

Case Studies Presented at Diabetes Technology Society Conference Highlight Millions in Hospital Cost Savings with Improved Glucose Control

One study presented at the International Hospital Diabetes Meeting observed first-year cost savings of nearly $7.5 million stemming from adoption of best practice for insulin therapy.

A series of studies and presentations included in the 6th International Hospital Diabetes Meeting highlighted the high cost of poor glucose management, and in particular, severe hypoglycemia (< 40 mg/dL) in the acute care setting. With thirty to forty percent of ALL hospitalized patients requiring insulin therapy, not just those with diabetes, the common problem of insulin mismanagement has significant impact on care quality, patient safety and patient experience, as well as cost and value.

Computer-Guided Insulin Titration Helps Hospitals Meet Basal Bolus Standard, Study Finds

An analysis of an electronic glucose management system developed by Glytec, which helps nurses safely manage basal bolus insulin dosing in the hospital, was presented at the recent meeting of the American Association of Clinical Endocrinologists (AACE) 26th Annual Scientific and Clinical Congress, held in Austin, Texas.

The movement toward quality ratings — and an increased emphasis on preventing readmissions — demanded solutions that help hospitals achieve the standard, which ADA clarified in January 2017 to state that basal bolus insulin was the preferred treatment for noncritically ill patients, and that, “Prolonged sole use of sliding scale insulin in the inpatient hospital setting is strongly discouraged.”

Hawaii Pacific Health Receives 2016 HIMSS Enterprise Davies Award Based in Part on Success With Glytec’s eGMS®

Our client, Hawaii Pacific Health, was named a HIMSS Enterprise Davies Award recipient based in part on its success implementing Glytec’s eGlycemic Management System® to support computer-based insulin dosing and a new hypoglycemia protocol.

The results were better glucose control with computer-directed insulin dosing system versus usual care. 39.8% of all glucose values in the ICU setting were above the recommended limit (glucose 180 mg/dl) with usual care. With computer-directed insulin dosing system for intravenous insulin, the hyperglycemia rate was much lower representing just 16.6% of all glucose values.

Putting Epic’s EHR to Work: Hawaii Pacific Executives on the Intersection of Health IT & Quality

We started on this journey because we identified patients with diabetes and their comorbidities as top reasons for readmissions and longer length of stay.

These patients are also at higher risk for postoperative complications. We looked at how our system was managing hyperglycemia. We looked at glucose values for all of our patients. The EHR makes that possible. Looking at the data, I was quite concerned. We had a lot of patients with hyperglycemia. There have been a lot of conversations at the American College of Surgeons National Surgical Quality Improvement Program meetings about hyperglycemia. Starting from there, we found a program [Glytec] to integrate into our EHR — something that could help with hyperglycemia management.

Woodridge Seeing Great Glycemic Results from Using Glucommander

As part of Mountain States’ Corporate Glycemic Initiative, they implemented Glucommander, a computerized insulin dosing software, to get a better handle on patient glucose levels.

The Mountain States psychiatric facility invariably has patients who need their blood sugar monitored, but people at Woodridge aren’t in the same clinically controlled environment as patients in most hospitals. They aren’t bed-bound with an IV in their arm, and nobody monitors exactly what, when and how much they eat. Thus, it’s harder to control their blood sugar levels.

Doctors and nurses following the standard of care in hospitals aren’t just paying more attention to analytics. These days, the algorithm can be the cornerstone of superior care. Consider the tricky process of controlling the glycemic levels of patients with diabetes. At eight-hospital, Virginia-based Sentara Healthcare, this translates into a 79% improvement over the national average for hyperglycemia rates. These hospitals have achieved an overall hypoglycemia rate of 0.83 percent, which hospital officials says is a remarkable blended rate for IV and subcutaneous insulin therapy.

Glucose Management System Improves Standard of Care at Sentara

Hospitals have often had trouble managing patients with hyperglycemia and hypoglycemia. To help remedy that problem, Sentara Healthcare has ushered in a new standard of care in glucose management.

In 2008, Sentara saw several of its patients die due to poor management of their diabetes. There were several devastating events of prolonged hypoglycemia that resulted in permanent brain damage for patients, says Paul Chidester, M.D., vice president of medical affairs at the Va.-based Sentara Norfolk General Hospital within the health system. Sentara had reached a point when it knew it needed to do something to improve the care of its diabetes patients. “We sort of tolerated poor control of hyperglycemia and viewed episodes of hypoglycemia as a rite of passage of being in the hospital,” Chidester admits. [We said] poor control happens, just live with it. We then realized the negative impact that had on our patients, and decided that it had to change.”
